Grow Obstetrics collects information for the primary purpose of providing quality health care. We require your personal details and a full medical history so that we may properly assess, diagnose, treat and be proactive in your healthcare needs. Information collected will be used in the following ways:
- For administrative purposes in running our medical practice.
- For billing purposes, including compliance with Health Insurance Commission requirements.
- For disclosure to others involved in your health care and/or claim. This may include treating doctors, specialists, allied healthcare personnel outside this medical practice and any third party involved in your case. This may occur via referral to other doctors or for medical tests and in reports or results returned to us following the referral.
- To conduct practice audit and medical research, in which case all data is analysed without identifying personal details. Audit and research are important ways of maintaining high standards of medical practice. Audit is a requirement of the Royal Australian and New Zealand College of Obstetricians and Gynaecologists.
You are not obliged to provide any information requested of you although failure to do so may compromise the quality of the health care and treatment given to you. You have the right to access information collected about you, except in circumstances where access may legitimately be withheld. You will be given an explanation in these circumstances.
If your information is to be used for any other purpose other than that set out above, your further consent will be obtained.
